What expenses qualify for funding?
Expenses that may be reimbursed or paid in advance to patients include but are not limited to: hotel expenses, gas or other travel expenses, services provided by KFMC providers that have an exorbitant cost, routine preventative services that KFMC providers are unable to perform, and services that are not covered by the existing KFMC sliding fee scale discount.
How much funding will I receive?
Funding by the foundation is based on patients household income. Through an information sharing system, when a patient applies for funding, their income status will be obtained from Kiamichi Family Medical Center. There are five categories for funding available.
What are your terms and conditions?
Patients much complete and submit an application to the board of directors. Patients must also submit any proof of expenses such as receipts or estimated costs of services if requesting prior to a service being performed.
